Payments
Payment for services and product must be paid in full once the service and/or product is provided. All payments will be collected in full at the conclusion of the scheduled visit.

 

Methods of Payment
Payments provided must be either cash, personal check, or credit card.

 

Prepaid Consults/Packages
Consults or packages that have been prepaid and not used are non-refundable.  Prepaid consults or packages that have expired are non-refundable and are not able to be used after their expiration date.

 

Consultation Length
Consultations may last anywhere from 1-3 hours in length depending on the purpose for the consultation, the material being covered, and any additional testing or measurements that are planned.

 

Online Consultations
Payment for long distance consults via Internet/Skype must be made prior to the consult taking place. Arrangement for payment for this type of consult must be prearranged with the Dietitian/Office Manager.

 

Insurance
I understand that Back To Balance runs a strictly cash based practice and that insurance of any kind is not accepted by Back to Balance. Please be aware that some insurance companies may be agreeable to reimbursing you for all or a portion of the fees but that remains your responsibility as a client to investigate whether this situation applies to you with your current provider. Back to Balance is happy to provide you with receipts/invoices of transactions completed for services rendered should they be requested by any insurance provider.

 

Lab Testing
Lab Testing is done on an “as needed” basis just as it is done in a physician’s office. Lab fees will vary based on individual needs for testing and based on the specific test or tests being completed. Some initial lab testing is usually done within the first few visits to establish baseline levels; additional lab testing is completed periodically as necessary throughout the time frame of working together to monitor improvement/changes. The most routine and common lab testing completed might include:

  • Functional Adrenal Stress Profiles (Circadian Rhythm, DHEA, Cortisol)
  • Hormone Profiles (Estrogen, Progesterone, Pregnenalone, Testosterone, Melatonin, DHEA, Cortisol)
  • GI Pathogen Screen (Parasites, Bacteria, Fungi)
  • Toxic Metal Screen (mercury, lead, aluminum, arsenic, cadmium)
  • Organix Comprehensive Metabolic Profile (Metabolic Assessment Profile which reveals level of functioning of metabolism (fatty acid, carbohydrate, protein), energy production (citric acid cycle), methylation, vitamin insufficiency, neural/neurotransmitter function, antioxidant status, oxidative damage, detoxification function, dysbiosis, bacterial/yeast overgrowth)
